I hereby certify that guest(s) listed on this form are over the age of 18 unless otherwise noted. Any guest not over the age of 18 will be accompanied by an adult at all times while using the Cedars Tennis Resort & Club facilities.

Guests will be required to complete our Waiver of Liability prior to using Cedars Club Facilities. All guests and rental guests must visit the Pro Shop upon arrival to finalize the registration process and to pay any necessary dues (unless fee has been pre-paid in advance by unit owner or guest).
